Supplements and Arthritis Medications: What to Know
Arthritis is one of the most common chronic conditions in the country. An estimated 58.5 million U.S. adults, or roughly 1 in 5, have some form of arthritis, according to the CDC.¹ Osteoarthritis is the most common type, but for people with rheumatoid arthritis and other autoimmune forms, treatment often looks different.
At the same time, roughly one in three American adults takes both prescription medications and dietary supplements simultaneously, according to a study using data from the National Health and Nutrition Examination Survey (NHANES).² And a 2018 survey conducted by Wakefield Research found that nearly 40% of Americans taking both prescriptions and supplements didn't know that vitamins and supplements could affect how their medications work.³
If you manage arthritis with NSAIDs, this matters in a specific way. NSAIDs already carry a meaningful risk of gastrointestinal irritation and bleeding on their own. Several popular joint-support and anti-inflammatory supplements work through mechanisms that can add to that risk, not because the supplements are dangerous by themselves, but because the combination compounds an effect that's already present.
Here's what you should know.
Why NSAIDs Are Sensitive to Supplement Interactions
First it helps to understand how NSAIDs affect the digestive tract so that you can then understand which supplements need extra caution.
Common NSAIDs like naproxen, ibuprofen, and diclofenac work by limiting cyclooxygenase (COX) enzymes, specifically COX-1 and COX-2, to reduce pain and inflammation. The problem is that COX-1 also produces prostaglandins that protect the stomach and intestinal lining. When NSAIDs suppress COX-1, that protective layer thins out, which is why NSAIDs are a leading cause of stomach irritation, ulcers, and GI bleeding.⁴,⁵ Studies estimate that endoscopic evidence of ulcers appears in 10-30% of regular NSAID users, with serious complications like bleeding or perforation in 1-2%.⁶
COX-1 inhibition has a second effect worth noting: it reduces thromboxane A2 production in platelets, which makes platelets less likely to clump together.⁷ In other words, NSAIDs don't just irritate the gut lining, they also make it harder for the body to stop bleeding if irritation occurs. This is the mechanism that connects NSAIDs to several common supplements.
Corticosteroids (prednisone, methylprednisolone), often used short-term for arthritis flares, work differently, through broad immune suppression and anti-inflammatory effects, and carry their own considerations around bone density and blood sugar with long-term use.
DMARDs and biologics (methotrexate, adalimumab, etanercept), used for rheumatoid arthritis, work by tuning the immune system directly and generally have less overlap with the bleeding-risk supplements below, though they carry their own interaction profile that's worth a separate conversation with your rheumatologist.
This means the interaction risk between a supplement and an arthritis medication depends heavily on which medication you're taking, and for how long.
Vitamins and Supplements That Require Caution With NSAIDs
Fish Oil (Omega-3 Fatty Acids): Additive Bleeding Risk at Higher Doses
Fish oil is one of the most commonly recommended supplements for joint health, and there's real research behind its anti-inflammatory benefits. But EPA and DHA also reduce thromboxane A2 production and platelet grouping, essentially the same mechanism NSAIDs use to thin blood.⁸ At typical doses, this usually isn't a significant concern. At higher doses, particularly above 3-4 grams daily, the antiplatelet effect can add to what NSAIDs are already doing, especially relevant for anyone also taking a blood thinner or aspirin alongside their NSAID.⁹
Turmeric (Curcumin): Antiplatelet Effects
Turmeric supplements are popular for joint support and inflammation, which makes sense given curcumin's anti-inflammatory properties. Curcumin also has demonstrated antiplatelet effects, meaning it can add to the bleeding risk NSAIDs already carry.¹⁰ As with the statin interaction, typical culinary use of turmeric in food is a low concern whereas the caution applies to concentrated curcumin supplements, especially at higher doses.
Ginkgo Biloba: Documented Bleeding Interactions
Ginkgo is one of the more well-documented bleeding-risk supplements. Research shows it can slow clotting, and case reports describe bleeding events in people taking ginkgo alone or in combination with NSAIDs.¹¹ A recent hospital-record analysis found ginkgo biloba among the supplements most frequently associated with drug interactions involving antiplatelets and NSAIDs.¹²
High-Dose Vitamin E
Vitamin E at supplemental doses has mild antiplatelet properties. It's a less prominent concern than fish oil or ginkgo, but it's frequently listed alongside them as a supplement to flag when someone is on both an NSAID and a blood thinner.
Ginger: Mild but Additive
Ginger, another common joint-support ingredient, has mild antiplatelet activity. On its own, this is not often a significant issue, but combined with an NSAID and one or two of the other supplements on this list, the effects can compound.
White Willow Bark: Aspirin-Like Effects
White willow bark contains salicin, which the body converts to a compound closely related to aspirin's active ingredient.¹³ Taking it alongside an NSAID is functionally similar to doubling up on a mechanism your medication is already using, with no added benefit and added risk.
Vitamins and Supplements Generally Considered Safe With NSAIDs
Glucosamine and Chondroitin
These are among the most studied joint-support ingredients, and research has found no significant direct interaction between glucosamine and common NSAIDs like ibuprofen or naproxen.¹⁴ The caution with these two applies specifically to people also taking a blood thinner like warfarin, since glucosamine and chondroitin can independently raise bleeding and bruising risk in that context.¹⁵ For someone on an NSAID alone with no blood thinner, they're generally considered safe.
Vitamin D and Calcium
Both are frequently recommended for people on long-term corticosteroids, since steroid use is associated with reduced bone density over time. Neither interferes with NSAIDs or corticosteroids and both are generally considered safe to combine.
Probiotics and Digestive Enzymes
Given strongly gut irritation is connected to the NSAID interaction story, this is one of the more relevant categories. Multiple studies have found that specific probiotic strains, including Bifidobacterium breve and Bifidobacterium longum, can help protect the intestinal lining against NSAID-induced damage.¹⁶,¹⁷ This doesn't offset the need for careful NSAID use, but probiotics don't add risk and may offer real protective benefit for people who take NSAIDs regularly.
Magnesium
Magnesium doesn't have a direct interaction with NSAIDs or most arthritis medications and is generally considered safe, though it's worth taking at a separate time from other minerals to avoid absorption competition.
Why Your Specific Arthritis Medication Matters
Not all arthritis treatments carry the same interaction profile. This is a point most supplement articles miss, but it matters:
Highest bleeding-risk overlap: NSAIDs (naproxen, diclofenac, ibuprofen), especially when combined with fish oil, turmeric, ginkgo, vitamin E, ginger, or white willow bark. The risk compounds further if you're also on a blood thinner or aspirin.
Bone density and blood sugar considerations: corticosteroids (prednisone, methylprednisolone). Vitamin D and calcium are commonly recommended alongside long-term use; blood-sugar-lowering supplements should be discussed with a physician if you also have diabetes.
Distinct interaction profile, less overlap with the list above: DMARDs and biologics (methotrexate, adalimumab, etanercept) for rheumatoid arthritis. These have their own set of considerations, including folate supplementation alongside methotrexate specifically, which is worth a dedicated conversation with your rheumatologist or pharmacist.
If you take an NSAID regularly alongside any of the supplements on the caution list, or you're combining an NSAID with a blood thinner, asking your pharmacist to review the full combination is a reasonable and often quick conversation to have.
What This Means for Choosing Supplements
If you manage arthritis with medication, the supplement you choose matters more than most brands acknowledge. A few practical tips:
Check the ingredients, not just the label claims. A "joint support" supplement might contain turmeric, ginkgo, or fish oil at meaningful doses, all of which can add to what your NSAID is already doing to bleeding risk. A "healthy aging" or "circulation support" formula might contain the same ingredients under different marketing.
Know which arthritis medication you take. Whether you're on an NSAID, a corticosteroid, or a DMARD changes which supplement interactions are actually relevant to you.
Pay attention to dose, not just ingredient. Culinary turmeric and a 1,000mg curcumin supplement are not the same exposure. The bleeding-risk supplements on this list are mostly a concern at supplemental, not dietary, doses.
Look for supplements that disclose interaction data. Choosing a brand that publishes pharmacist-reviewed interaction information gives you a concrete starting point for the conversation with your doctor.
